|Title of the Programme||Children and Youth at Risk|
|Title of the Project||Transformation of care for vulnerable children and youth|
|Number of the Project||EHP-CZ04-OV-1-005-01-2014|
|Objective of the Project||
The aim of the project is to prepare transformation plans, which will cover all the residential facilities established by the PR, i.e. Holice Children’s Home, Husova 623, Pardubice Children’s Home, Dolní Čermná Children’s Home, Polička Children’s Home, Moravská Třebová Children’s Home and the so-called “special healthcare facility” – the Veská children’s centre, the Children’s Home for children from the ages of 1 to 3 in Holice, and Svitavy children’s centre.
The objective of the project is to draw up transformation plans for 8 facilities for vulnerable children established by the PF, with the aim of reducing the residential capacity of these facilities by at least 50%, to develop existing services and to create new services for vulnerable children and their families in response to the needs identified and in accordance with legislation in the area of the social and legal protection of children. In the case of children’s homes, this will involve the preparation of a comprehensive plan to transform residential care for children into an effective network of outpatient and outreach prevention and support services. The transformation plan will result in the gradual transformation of activities provided by residential facilities into field activities and outpatient care, while continuing to make use of their professional potential. The purpose of the new concept of care for vulnerable children is to minimise the numbers of children admitted to residential facilities. Those children who are admitted to residential facilities will receive individual care and their stay will be reduced to only such period as is absolutely necessary. The objective of the transformation plans for special healthcare facilities is primarily to assess the need for and capacity of these facilities, to minimise the number of beds allocated to children unaccompanied by family members and to create sufficient residential capacity to enable children to stay in the residence in the company of a family member. Another objective is to create capacity for so-called “relief care” for vulnerable children from both foster and biological families.
